Terms and Conditions

These terms and conditions relate to the Program you are enrolling in. Game On Sports Clinic Pty. Ltd. owns and manages under the Game On branding. 

These terms and conditions are for Game On and by accepting the terms and conditions below, you are entering into a legal agreement with Game On. 

I agree to enrol my Child/ren with Game On to provide programs and activities for my Child/ren. I declare that the information I have provided on the enrolment form is true and correct and I have completed the enrolment form to the best of my knowledge regarding all aspects of my Child/ren including any medical conditions and allergies. I agree to update my details and enrolment online if any of the information on my enrolment form changes in the future. 

1. I confirm that I have the lawful authority and capacity as the Child/ren’s legal Parent or Guardian to enrol my Child/ren with Game On. I will immediately notify and provide a copy of any order a court makes for the custody and care of the Child/ren I have enrolled with Game On. 
2. I acknowledge that Game On operates at the venue stated in the enrolment form and this form is also valid for any other Game On Programs my Child/ren attend. 
3. I confirm that my Child/ren is at least 5 years of age at the time of Game On attendance.  
4. I confirm that my Child/ren has completed a minimum of 6 months of Prep schooling or equivalent unless agreed upon with the Director in writing prior to the commencement of the Program. 
5. I give permission and consent for my Child/ren to leave the venue were advised by an Employee or in the case of a medical emergency and emergency drills. I also give permission and consent for an Employee to seek transportation of my Child/ren by an ambulance if/as required. 
6. I give permission for my Child/ren to be photographed for Game On publicity (including but not limited to social media). I can deny permission when completing the Game On Child Information online form. 
7. I give permission for my Child/ren to swim at Game On. I can deny permission when completing the Game On Child Information online form. By consenting to my Child/ren swimming at Game On, I am acknowledging that the Child/ren are capable of swimming or floating in deep water and are able to participate safely in a pool, in a group setting, under the supervision of a lifeguard with the ratio of supervisors to children as per the Game On policies and procedures. 
8. I give permission for my Child/ren to participate in the program-based activities organised by Game On for the days my Child/ren attend Game On. 
9. I give permission for my Child/ren to participate in program-based activities for the days my Child/ren attend Game On and give permission for my Child/ren to participate in and view G and PG rated content used. 
10. I understand that my Child/ren will be under the direction and control of Employees who will follow the Game On policies and procedures (including behaviour guidance procedures if required). 
11. I understand that if my Child continues to demonstrate unacceptable behaviour after behaviour guidance procedures have been followed, I will be notified by Employees and may be required to collect my Child/ren. I agree to immediately make arrangements to collect my Child/ren from Game On. I acknowledge that Game On may cancel my Child/ren’s enrolment. 
12. I authorise Employees to apply sunscreen/insect repellent to my Child/ren when required, unless I have notified Game On in the Game On Child Information online form that sunscreen/insect repellent is not to be applied. 
13. In the event that the parent/guardian is unable to collect the Child/ren from Game On, I agree that I will be required to provide written confirmation that an alternative person will be collecting the Child/ren from Game On. I agree that a Game On Employee may contact the parent/guardian to confirm that the non-parent/guardian is authorised to collect the Child/ren. 
14. I agree that my Child/ren will be wearing sunscreen, a hat, closed in shoes with adequate grip and a shirt with sleeves upon arrival. Failure to do so may require parents/guardians to return with required items as soon as possible.  
15. I acknowledge that my Child/ren may not be able to participate in outdoor activities unless I provide and they wear a hat. 
16. I acknowledge that my Child/ren may not be able to participate in outdoor activities unless I provide and they wear closed in shoes with adequate grip. I acknowledge that no tags are to be attached to shoes.  
17. I acknowledge that my Child/ren may not be able to participate in swimming activities unless I provide a towel and high visibility swimming attire, a long sleeved rashie/swim shirt to provide sun protection and a swimming cap if my child/ren has long hair, and any flotation devices required. 
18. I acknowledge that my Child/ren may not be able to participate in outdoor activities unless I provide a water bottle.  
19. I acknowledge that my Child/ren may not be able to participate in outdoor activities unless all jewellery is removed. 
20. I acknowledge that my Child/ren may not be able to participate in outdoor activities if fingernails are not cut short.   
21. I take full responsibility for my Child/ren’s belongings while they are at the Game On. I have labelled all of my Child/ren’s belongings with my Child/ren’s first and last name. 
22. I acknowledge that Game On will not accept my Child/ren if my Child/ren is suffering from an infectious or contagious disease or in the opinion of Employee is too unwell to attend Game On I agree to immediately make arrangements and collect my Child/ren from the venue if requested by Employees. I agree that I will not bring my Child/ren back to Game On until 24 hours after the illness or disease has passed (or until I have produced a medical certificate if requested by Employees). 
23. I agree that I will sign in my Child/ren into Game On upon arrival. I agree that I will sign out my Child/ren from Game On upon collection. I acknowledge that my Child/ren is not under the supervision of Game On unless they are appropriately signed in per processes. I acknowledge that my Child/ren are required to be signed in and signed out between 8am and 4pm each day.  Child/ren 10 years or older may be able to sign themselves in/out, if agreed with the Director in writing prior to the commencement of the Program.  
24. I give permission for and consent to my Child/ren being given basic first aid treatment in the event of an accident, incident or illness.  
25. I agree to collect my Child/ren if an Employee requests due to an accident, illness or if he/she becomes unwell at Game On. I give permission for Employees to seek and arrange medical or emergency medical treatment and/or transport if my Child/ren requires it from a medical practitioner, hospital or ambulance service. I consent to Game On releasing my Child/ren into the care of a medical practitioner, ambulance service representative, hospital or other medical personnel as deemed necessary. I agree to cover the cost and expense of any medical treatment and authorise Game On to deduct as a fee any amounts paid by Game On, by way of reimbursement. 
26. I agree that as a Guardian or by adding an Emergency Contact I am making this person an authorised nominee who has been given permission to collect my Child/ren from Game On. 
27. I agree that as a Guardian or by adding an Emergency Contact I am making this person an authorised nominee who has been given permission to authorise consent to medical treatment, administration of medication, take my Child/ren out the Program 
28. I agree that as a Guardian or by adding an Emergency Contact I am making this person an authorised nominee who has been given permission to authorise Game On to transport, or arrange transportation of, my Child/ren. 
29. I agree and acknowledge that Game On does not accept any liability for personal injury, property damage or loss sustained by any Child/ren due to them participating in a Program unless the injury, damage or loss was caused by the proven negligence of Game On its directors or employees, or is otherwise required by law and I release Game On from all claims I or my child may otherwise have in respect of any such personal injury, property damage or loss. 
30. I indemnify Game On (including its directors, officers and employees) against any costs, expenses or liability incurred as a result of any damage or injury caused by my Child/ren to: 
    • property at the venue and 
    • any other Child/ren and Employees at Game On I agree to update and/or change my details and bookings via the online system using my user name and password when required, advised and at the beginning of each Program period.  
31. I agree to abide by the cancellation procedures and will inform the Employees at Game On of any absence of my Child/ren. I will be charged for the Program in accordance with the refund policy or if I do not follow the refund policy procedures.  
32. I agree that Game On reserves the right to no longer allow children to attend Game On, if fees are not paid on time, or if there is any money owed to Game On for previous attendance.  
33. I understand that if I wish to cancel or make any changes to my Program bookings without being charged, I’ll need to ensure that they are made online 48 hours prior to the dates I require my Child to attend. Cancellations will not be accepted after this date. This will be administered in accordance with the Game On Refund Policy.  
34. I acknowledge that Game On may amend and update its policies, regulations or terms and conditions from time to time and the amended document can be viewed on the Game On website or via a printed copy on request. I acknowledge that it is my responsibility to ensure that I am familiar with amended policies and procedures. I agree to comply with all amended policies and procedures.

Definitions 

• Activities means events held on a clinic day. 

• Child/ren means any child who is enrolled in and attending Game On for the relevant period. 

• Daily Rate means the rate that was paid at the time of purchase. 

• Director means Sebastian Pelly. 

• Employee means a person employed by Game On or the Director of Game On. 

• Game On means Game On Sports Clinic Pty Ltd. 

• Program means a clinic day.
